When you opt in for Pfizer Dedicated Care Coordinator (DCC) support you will be contacted by a Pfizer DCC who can help you understand your insurance benefits and navigate the process to access your prescribed medication. Pfizer DCCs are field based employees of Pfizer, and if you choose, will help answer questions you may have about accessing the medication prescribed by your Healthcare Provider. The Pfizer DCC assigned to you serves as a resource for you on your journey to starting therapy and is familiar with access and reimbursement requirements, financial offerings, and baseline prescreening tests. Should you request baseline prescreening testing assistance, the Pfizer DCC will verify your eligibility, including contacting your identified physician regarding your request. Working with a Pfizer DCC is optional. By completing this form, I request Pfizer DCC support and agree to receive telephonic communications at the number provided (wherever provided) from the Pfizer DCC assigned to my case as described above. I understand I can opt-out of support from communications with the Pfizer DCC at any time by informing my assigned DCC that I no longer wish to communicate with them. I understand that this form will remain in effect for one year (i.e., 1 year from the date of my signature) unless I opt out of support and communications with the Pfizer DCC at any time by informing my assigned DCC that I no longer wish to communicate with them.
